What insurance, Health Plan (HMO-PPO) does Vermont HealthCare Center accept?
Health care can certainly seem confusing at times. To select a physician or find out which plans we accept, please call (323) 660-5624. When you call we will answer your questions and help you choose a plan.
We accept more than 20 health insurance plans. Whether you have an HMO, PPO or Medicare health plan, we are will most likely accept your insurance.
We provide on-site services for blood draw, urine studies, electrocardiograms, ultrasound, pulmonary testing and nerve conduction studies and X-rays. You may be given special instructions for lab work or diagnostic studies. For example, you may be asked to arrive a half hour before your appointment or asked to not eat after midnight the day prior to your appointment.
Please notify your physician at the time of your appointment if you need a prescription refill. If you have an authorization request or you are in between appointments and need a refill, have your pharmacist call our office at (323) 660-5624 and we will handle the matter directly with them.
Request for Medical Records/Forms
Please allow one week for the completion of forms such as disability, authorizations, etc. Completion of forms requires your nurse and doctor to review medical records. A fee (depending on the type of form) is charged in most cases.
